What to bring.

A smooth check-in starts at home. Have these three things ready before you walk through the door.

Step 01

Photo ID

A current driver's license or government-issued photo ID. We'll make a quick copy at check-in for your file.

Step 02

Insurance card

Your primary insurance card, plus any secondary or supplemental coverage. We'll verify benefits before your visit when possible.

Step 03

Imaging & records

Recent X-rays, MRI, or CT scans on CD or printed report. Helpful — but not required — for your first visit.

How quickly can I be seen?

Most new patients are scheduled within the same week, and urgent cases are prioritized. Let our team know the situation when you call and we'll find the earliest opening.

Which insurance plans do you accept?

We accept the vast majority of PPO and HMO plans across the region — covering roughly 99% of patients we see. If you're unsure about a specific plan, our team will verify it on the call.

Do I need a referral from my primary care doctor?

It depends on your insurance plan. HMO plans typically require a referral; PPO plans usually don't. If you're not sure, we'll help you figure it out when you call.

What if my situation is urgent?

For severe pain, a recent injury, or anything that can't wait, call us first thing in the morning and tell our team it's urgent. For emergencies, please call 911 or go to the nearest emergency room.
